The Transition Program assists in the monitoring of adolescents who have recently completed placement or serves as an alternative to placement.
The transition referral plan is formulated by the referral source with input from the parents/legal guardians, the adolescent, and other professionals involved in the matter. The plan identifies areas of need such as education, counseling, chemical dependency and social behavior issues to be addressed.
The Transition Program provides services for adolescents from the ages of 12 to 17 years. Referrals are accepted through Social Service agencies and Probation Departments of theTri-County area. The Transition Program provides adolescents with connections and referrals to supportive community services and assists in home school district reintegration as well as supportive counseling.
